- Population
- Clinician-directed adult arithmetic in exactly one of two narrow contexts: second-line androgen-status assessment in a symptomatic male after appropriately collected or repeated total testosterone, or androgen-excess assessment in a nonpregnant female. It requires age 18 or older and three final quantitative serum results from the same draw and laboratory: total testosterone in nmol/L by a validated LC-MS/MS method, SHBG in nmol/L by an explicitly validated method, and measured albumin in g/L with an explicit validated assay identity. Every result must be within its analytical measuring range and retain collection-date, laboratory, method, analyte, matrix, unit, and status provenance.
- Limites et exclusions
- Pregnancy; age under 18; acute illness; suspected binding-protein variant; competing or exogenous steroid contexts; oral testosterone, DHT, or mesterolone exposure; treatment monitoring; assumed, defaulted, stale, mixed-patient, mixed-draw, censored, qualitative, preliminary, analytically out-of-range, non-serum, unit-converted, or method-unverified inputs. · Substitution of equilibrium-dialysis measurement, another calculated-free-testosterone model, a fixed or default albumin value, alternative binding constants or stoichiometry, the Södergård, Ly, Zakharov, or modified-Vermeulen model, bioavailable testosterone, or a different laboratory calculator or range. · Reference intervals, cutoffs, flags, categories, normal or abnormal labels, diagnosis or exclusion of androgen deficiency or excess, probability, prognosis, treatment eligibility, monitoring target, dosing, management, recommendation, or comparison with another laboratory's reference interval or calculator. · The value is a modelled estimate rather than measured free testosterone. Published comparison with standardized equilibrium dialysis found systematic overestimation, and SHBG platform bias can materially move the estimate; assay- and method-specific results are not portable.
