- Current US OPTN Policy 9.1.D adult-at-registration MELD arithmetic, policy snapshot 2026-06-24 checked 2026-08-12: 1.33 if sex for purposes of adult MELD calculation is female + 4.56×ln(total bilirubin mg/dL) + 0.82×(137−sodium mEq/L) − 0.24×(137−sodium)×ln(bilirubin) + 9.09×ln(INR) + 11.14×ln(creatinine mg/dL) + 1.85×(3.5−albumin g/dL) − 1.83×(3.5−albumin)×ln(creatinine) + 6; bilirubin, INR, and creatinine below 1 are set to 1; creatinine above 3 or qualifying dialysis/CVVHD is set to 3; sodium is limited to 125–137 and albumin to 1.5–3.5; natural logs, no intermediate rounding, final arithmetic half-up whole number limited to written-policy range 6–40
- Population
- US liver-transplant candidates who were at least 18 years old at this waitlist registration and for whom the transplant program, in consultation with the candidate and clinical team, independently established the policy field sex for purposes of adult MELD calculation. Exact final total serum bilirubin, serum sodium, INR, serum creatinine, serum albumin, official units, applicable laboratory dates and context, and the two policy dialysis/CVVHD criteria relative to the creatinine test must be independently verified. This arithmetic does not establish whether laboratory certification, recertification, or the same-two-day voluntary-change rule is satisfied.
- Limites et exclusions
- Candidates who were younger than 18 at waitlist registration, including candidates now aged 18 or older; the separate adolescent branch with constant 7.33 and no adult sex term; PELD; or any inferred registration-age branch. · Inferring the sex-for-adult-MELD category from birth sex, gender identity, name, anatomy, laboratory values, or therapy; an unknown category or dialysis/CVVHD history; missing albumin; imputed, carried-forward, mismatched, uncertified, or unverified values or units. · Calling this output an authoritative allocation MELD, allocation priority, candidacy or eligibility decision, Status 1A/1B, exception score, liver-intestine point total, certified or current OPTN record, match-run result, mortality probability, risk category, diagnosis, prognosis, or treatment recommendation. · Delaying, withholding, or altering indicated albumin, dialysis, CVVHD, anticoagulation, or other care to influence score arithmetic; any treatment, listing, allocation, management, or disposition decision. · The uncapped 2021 research MELD 3.0 score above 40. This implementation follows the current written OPTN adult policy's 6–40 limit and is not interchangeable with the uncapped research output.
