- Oken et al. official 1982 ECOG Performance Status Scale documentation contract, engine ecog-performance-status-1982-documentation-v1: this helper records, but never calculates, derives, converts, recommends, or imputes, one qualified oncology clinician-selected grade from the complete official English descriptors at one identified assessment timepoint. A living assessment accepts only grade 0, 1, 2, 3, or 4; grade 5 means Dead and is accepted only through the separate death-already-documented branch after independent verification of an authoritative clinical death record. Grade 4 is never treated as death. Exactly 50% of waking hours and any borderline or discordant descriptor fit remain clinician judgments; work in the source descriptors means functional capacity, not employment status. Four common confirmations are required: adult oncology documentation scope; selection of the official 1982 scale; one documented assessment or death-record timepoint; and acknowledgement of subjectivity, cause non-differentiation, and documentation-only limitations. A living assessment additionally requires five independent confirmations: qualified-clinician selection; review of the complete descriptor domains without a single-metric shortcut; no algorithmic derivation, Karnofsky conversion, or imputation; review of potential causes, confounders, and support context without automatic adjustment; and clinician resolution of borderline or discordant fit. The death-record branch instead requires two independent confirmations: verification of the authoritative clinical death record; and acknowledgement that grade 5 means death only and was not inferred from living function. Output is limited to version, the unchanged grade, its exact official English descriptor, and the entered branch/timepoint/grade/identity audit; no classification, conversion, diagnosis, eligibility determination, prognosis, outcome estimate, treatment, dosing, disposition, management, or recommendation is supplied.

- Qualified oncology clinicians documenting one directly selected ECOG Performance Status Scale grade for an adult in an oncology context at one identified initial-oncology, pretreatment-baseline, on-treatment, post-treatment-follow-up, or other documented oncology assessment, or documenting grade 5 only from an independently verified authoritative clinical death record. The 1982 Oken publication presented standardized toxicity and response criteria for cancer clinical trials; it was not a reported derivation or validation study and provides no scale-development sample size, age cutoff, tumor-specific cohort, or recall window. Restriction to adults is this product's deliberately narrower policy, not an original ECOG source boundary. The complete official English descriptors remain authoritative; French text is convenience translation only.

- Pediatric grading; patient- or caregiver-selected grading; self-assessment; automatic chart abstraction; checklist, activity-question, waking-hours-percentage, employment-status, or single-metric classification; KPS, PPS, Lansky, or another performance-scale conversion; app resolution of exactly 50% of waking hours; cause attribution; or a missing, unknown, inferred, imputed, defaulted, carried-forward, mixed-timepoint, or algorithmically derived grade. · Grade 5 without an independently verified authoritative clinical death record, or inference of death from ECOG grade 4, complete disability, bed or chair confinement, hospice or palliative status, a moribund description, unresponsiveness, absent follow-up, or any other living functional observation. Grade 5 means death only. · Diagnosis; individual prognosis; survival or mortality estimation; outcome or toxicity prediction; trial inclusion or exclusion; treatment eligibility or fitness; treatment selection; dosing; disposition; monitoring; management; recommendation; or any care decision. · Presenting a French convenience translation as official; replacing or obscuring the exact official English descriptors; omitting the formal ECOG Performance Status Scale name above screen, print, or copied output or the full Oken citation and exact ECOG/ECOG-ACRIN credit line below it; reusing the journal article, ECOG-ACRIN logo, website design, colors, branding, or visual identity; or implying ECOG/ECOG-ACRIN endorsement.
