Version de la méthode: CKD-EPI 2012 cystatin-C-only eGFRcys (race-free; 0.9962 age-coefficient precision carried in Inker 2021 Table 2): eGFRcys = 133 × min(Scys/0.8, 1)^-0.499 × max(Scys/0.8, 1)^-1.328 × 0.9962^Age × 0.932 [if female]; race-free CKD-EPI 2021 creatinine–cystatin-C eGFRcr-cys: eGFRcr-cys = 135 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^-0.544 × min(Scys/0.8, 1)^-0.323 × max(Scys/0.8, 1)^-0.778 × 0.9961^Age × 0.963 [if female], where κ = 0.7 female or 0.9 male, α = -0.219 female or -0.144 male, Scr is standardized serum creatinine in mg/dL, Scys is standardized cystatin C in mg/L, and creatinine in µmol/L is normalized by dividing by 88.4
Population: Adults age 18 years or older represented by the CKD-EPI 2012 cystatin-C development and validation cohorts or the CKD-EPI 2021 combined-equation source cohorts, with the sex coefficient input verified exactly as defined by the source, cystatin C standardized to ERM-DA471/IFCC, and units verified. The combined equation additionally requires IDMS-traceable standardized serum creatinine and creatinine and cystatin C measurements from the same clinical interval with sufficiently stable biomarkers.
Limites et exclusions: The creatinine-only CKD-EPI 2021 equation, which is handled by the existing GFR calculator; pediatric populations and pediatric equations. · Unstandardized or unverified assays; cystatin C not traceable to ERM-DA471/IFCC; creatinine not traceable to IDMS for the combined equation; unverified units or source-defined sex coefficient input; unstable biomarkers; or combined creatinine and cystatin C from mismatched clinical intervals. · CKD stage or category, CKD diagnosis or confirmation, prognosis, risk estimation, drug dosing, deindexing to absolute GFR, treatment, management, or any clinical decision.
